Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />June 20, 2024 <br /> Facility Address: <br /> 4025 N Wilson WAY, Stockton <br /> Facility Name: <br /> Jarvis Kustoms <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />Emergency Response/Contingency Plan incorrectly marked automatic/electronic equipment shut-off system, <br />automatic fire suppression system and "other" with no specification. Section G of the Consolidated Emergency <br />Response/Contingency Plan listed a phone number instead of the location of telephones. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release <br />or threatened release of hazardous materials, including, but not limited to, the following: - immediate notification of <br />local emergency personnel and the Environmental Health Department (EHD) - procedures for the mitigation of a <br />release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br />evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: This was corrected on site. An updated Consolidated Emergency Response/Contingency <br />Plan into the California Environmental Reporting System (CERS) website. <br />This is a repeat violation (Class 2) Violation. <br />CA Health and Safety Code 25505(a)(4) 1020002 - Provided initial and annual training and maintained training records <br />for a minimum of three years. <br />OBSERVATION: Training records for the past three years were not observed on site . Per owner, safety meetings are <br />conducted but documentation of the trainings are not kept. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br />initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br />threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br />procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br />unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br />harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br />notice, for the business site. This training shall be documented electronically or by hard copy and shall be made <br />available for a minimum of three years. <br />CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br />training records to the EHD. <br />This is (Minor) Violation. <br /> 14 <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were four violations during the <br />inspection (two corrected on site). Complete and submit a copy of the Return to Compliance Certification form to <br />the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, <br />and any supporting paperwork, by 7/20/2024. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign.
