<br /> Stericycle accepts medical waste generated in a broad range of medical,diagnostic,therapeutic

<br /> and research activities. The term"medical waste"includes bio-hazardous,biomedical,infectious or Trace Chemotherapy Contaminated Waste-RCRA Empty drug vials,syringes and needles,spill kiM IV tubin!gand bags,
<br /> regulated medical waste as defined under federal,state or local laws,rules,regulations and guidelines, contaminated gloves and gowns,and related materials as defined in applicable laws,rules,regulations or guidelines
<br /> Except as defined by Publication 52 regulations,this excludes RCRA pharmaceuticals,all DEA scheduled
<br /> drugs including'controlled substances, bulk chemotherapy,waste containing mercury or other heavy
<br /> metals,batteries of any type,cauterizers,non-infectious dental waste,chemicals such as solvents,
<br /> reagents,corrosives or ignitable materials classified as hazardous waste under Federal and State EPA •Pathological Waste-Human or animal body parts,organs,tissues and surgical specimen(decanted of formaldehyde,
<br /> Regulations. In addition,Stericycle cannot accept bulk liquids(trace liquids must be contained via formalin or other preservatives as required by law)

<br /> Amalgam/Mercury Waste means(i)Dental Amalgam and materials containing dental amalgam(ii)
<br /> -Radioactive Waste Any container with a radioactivity level that exceeds regulatory or permitted limits,lead-containing materials
<br /> capsules,(iii)Amalgam dental traps,sludge from traps(iv)clean teeth,(Q Mercury Thermometers
<br /> (broken or unbroken)(viii)Breakers,Relays,Switches and Thermostats.Non-Acceptable Materials -Complete Human Remains(including heads,full torsos and fetuses)
<br /> means(i)Cyanides or cyanide-containing materials,(ii)Reactives,(iii)Flammables or Combustibles,(iv) -Bulk Chemotherapy Waste
<br /> Mercury cleanup materials containing Mercury and Mercury Amalgam,(v)Batteries and Light bulbs -Compressed Gas Cylinders,Canisters,Inhalers and Aerosol Cans
<br /> containing Mercury,(vi)Organic Mercury Components,(vii)Activated Carbon containing Mercury,(viii) -Any Mercury Containing Material or Devices:Any mercury thermometers,Sphygmomanometers or lab or medical devices
<br /> Improperly segregated,bulging,wet or leaking containers,or containers with Mercury product in excess
<br /> -Mercury-Containing Dental Waste Non-contest and contact amalgam and products,chairside traps,amalgam sludge or
<br /> of I pound and(ix)Infectious materials such as medical waste and sharps or any Regulated Medical

<br /> WASTE SEGREGATION AND PACKAGING
<br /> The generator is solely responsible for propery segregating,packaging and labeling of regulated medical
<br /> waste.Proper segregation and packaging reduces the potential for accidental release of the contents
<br /> Acceptable Materials List Non Acceptable Materials List
<br /> and exposure to employees,USPS workers and the general public.USPS regulations require that all
<br /> packages of regulated medical waste be prepared for transport in containers meeting the following -Dental amalgam -Cyanides or cyanide-containing materials
<br /> requirements under the USPS Publication 52 and per the specifications of the packaging approved -Materials containing dental amalgam -Reactives
<br /> by the USPS.It is imperative that generators follow the package closure and shipping document
<br /> -Amalgam capsules -Flammables or combustibles
<br /> requirements are adhered to ensure that mailback packages are not rejected or returned.Note:Loose -Amalgam dental traps -Organic Mercury components
<br /> sharps ARE NOT permitted in any wide mouth(pail type)mailback containers.All regulated medical
<br /> waste must be accompanied by a properly completed shipping document as provided in the kit. -Sludge from dental traps-If disinfected -Mercury-contaminated soils.
<br /> -Teeth-if disinfected -Activated Carbon containing Mercury
<br /> MANAGEMENT OF NON—CONFORMING WASTE -Intact mercury containing devices -Infectious materials such as medical waste and sharps
<br /> As required by regulation and company policy,USPS reserves the right and may refuse transport of any
<br /> containers that appear to be non-conforming because of their contents or are improperly packaged, APPLICABLE LAWS REQUIRE NO MORE THAN 1 POUND OF MERCURY PRODUCT MAY BE PACKAGED IN A CONTAINER
