SECTION III <br /> PROCEDURES FOR PROTECTING CLEAN INSTRUMENTS AND STERILE <br /> INSTRUMENT PACKS FROM EXPOSURE TO DUST AND MOISTURE DURING <br /> STORAGE <br /> After sterilization, describe the location where the packaged instruments are stored: <br /> Packaged items are removed with clean,gloved hands and placed in designated drawers in <br /> each procedure room. Drawers are reserved for sterile items only and to remain closed when <br /> access is not necessary. <br /> Is each peel pack evaluated at the time of storage and before use? YES X NO <br /> Describe the procedure followed if a sterilized package has been compromised: Any package <br /> that is found to be compromised in any way will be removed from the procedure area and <br /> returned to the sterilization room. The storage drawer will also be inspected to ensure that <br /> there is no possibility of further contamination to other packages. <br /> If disposable, single use, pre-sterilized instruments are used, a record of purchase must be <br /> maintained for a minimum of 90 days after use. Where are these records maintained? <br /> All records of disposable products will be filed and retained at the manager's desk. <br /> If the above instruments are used, a log must be kept of all procedures,the practitioner <br /> performing the procedure, client name, and date of procedure. Where are these records <br /> maintained? Information will be recorded with each consent form per procedure and will be <br /> retained at the manager's station for 90 days. After 90 days, records will be <br /> moved to storage closet and retained. <br /> Page 7 of 18 <br />
