OBSERVATION: An accurate UST Monitoring Site Plan was not submitted. The annular sensor was located on the wrong side of the tank. REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if installed: 1) monitoring panels; 2) in -tank liquid level probes (if used for leak detection); 3) devices monitoring tank annular spaces or vault; 4) devices monitoring product piping; 5) devices monitoring fill piping; 6) devices monitoring vent piping; 7) devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) devices monitoring under -dispenser containment; 10) line leak detectors; and 11) devices monitoring any other secondary containment areas. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation (Class 2) Violation. 10 CA Code of Regulations 2638(c), 2638(d) 2030074 - Annual Monitoring System Certification Form to the CUPA within 30 days of completion of the test. OBSERVATION: A monitoring system certification and leak detector testing was performed on 11 /15/23 A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. CORRECTIVE ACTION: Immediately provide this report to the EHD. This is (Class 2) Violation. 56 CA Code of Regulations 2716(a -e) 2030013 - DO inspections completed at least every 30 days and meet all requirements (TCR 12). OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The reports contain an incorrect CERS ID. -The 2/2/24 and 3/1/24 reports had a blank section V and were not signed by the owner/operator.
