The following items must be completed <br /> prior to the final inspection. <br /> 1. Prior to the inspection,you are to obtain a facility permit and an artist permit from the Environmental Health <br /> Department. The fee for the facility permit will be prorated to$116.50 and the fee for the artist permit will be <br /> $152. Both permits will be valid until June 30,2020. <br /> 2. Once obtained,the facility permit,artist permit, and current bloodborne pathogens certificate must be posted <br /> in the facility. <br /> 3. The absorbent areas of the wall must be painted. <br /> 4. A lined trashcan must be provided. <br /> 5. A properly labeled sharps container must be provided. <br /> The documents submitted require the revisions listed below. <br /> Client Consent Form: <br /> 6. Add a statement that the client is at least 18 years of age. <br /> 7. Add a description of the procedure such as"Microblading is a tattooing technique that uses a small handheld <br /> tool made of several tiny needles to add semi-permanent pigment to skin." <br /> Aftercare Instructions: <br /> 8. Add a description of the signs and symptoms of an infection such as"Signs and symptoms of infection, including, <br /> but not limited to, redness,swelling,tenderness of the procedure site, red streaks going from the procedure site <br /> towards the heart, elevated body temperature,or purulent drainage from the procedure site." <br /> 9. Add a statement indicating when a client should seek medical care such as"If you experience any of these signs <br /> or symptoms, seek medical care. <br /> Medical Questionnaire: <br /> 10. Add the question: Are you currently taking any medications?" <br /> 11. Change "take medication before dental work" to "Do you require antibiotics prior to surgery or dental work?" <br /> 12.
