The practitioner shall wear disposable gloves on both hands when touching, decontaminating, or handling a <br /> soiled object, instruments,jewelry or cleaning surfaces in the procedure, decontamination and sterilization areas. <br /> The surfaces and objects in the procedure area shall be disinfected before performing body art in that area. Inks, <br /> pigments, soaps, and other products in multiple-use containers shall be dispensed in a manner to prevent <br /> contamination of the storage container through the use of a single-use receptacle. HSC 119309 <br /> 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and <br /> washable. The body art facility shall be free of insect and rodent infestation. The procedure area must be <br /> separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to <br /> body art. Approval of co-located body art facilities are at the discretion of the LEA.-Aprocedure area must-be <br /> equipped with adequate light to perform aprocedure. -26i p� _ `re area shall have lined waste containers. <br /> HSC 119314& 119317 <br /> 33. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall <br /> be in good repair and capable of being disinfected. All counter surfaces and service trays in the procedure area <br /> shall have a smooth, durable, and nonabsorbent finish. HSC 119314 <br /> 34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A <br /> written Infection Prevention and Control Plan (IPCP)shall be maintained, followed and updated. HSC 119313 <br /> Submit a copy of an Infection Prevention and Control Plan to the EHD with evidence of training for the <br /> artist. Retain a copy in the facility. <br /> 37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art <br /> without a valid registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days <br /> of resignation, termination of a practitioner.A new hire or apprentice must be registered the EHD prior to <br /> performing body art at the facility. HSC 119312 & 119306 <br /> Obtain facility and artist permits prior to opening. <br /> All corrections are due prior to opening. <br /> Submit copies of the client consent form, medical questionnaire, aftercare instructions, and the Infection <br /> Prevention and Control Plan to the EHD prior to appointment for final inspection to open. <br /> Notes: <br /> 1. The facility will only employ one artist. <br /> 2. No client furniture was present at the time of the consultation. <br />
