The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for CITY OF STOCKTON MUD WELLS-3R as of September 09, 2024. <br />Open violations from May 10, 2024 inspection <br />Violation #3 - 1010017 - Business plan readily available to site personnel responsible for emergency <br />response or training. <br />OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon request. <br />Employees do not have access to the Emergency Response Plan and the inventory information on-site <br />REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required by <br />this article, and the site map required by this section shall be readily available to personnel of the business or the <br />unified program facility with responsibilities for emergency response or training pursuant to this section. <br />CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: A site map was not completed and submitted as part of the business plan due to missing the <br />following elements: <br />- No loading area indicated <br />- No evacuation staging area <br />- No Access/Exit points <br />REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs , evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. If a site map element is not applicable for <br />your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 2
