HSC 117960 - LQG shall file MWMP with the EA. <br />OBSERVATION: The Medical Waste Management Plan (MWMP) is inaccurate. The hauler address and treatment facility <br />locations reported in the MWMP do not match the facility’s disposal records. From February 2025 to current, the facility has <br />been shipping their medical waste to 7875 RA Bridgeford St, Stockton, CA 95206 and 1655 Milan Drive, Mccarran, NV 89437. <br />The hauler address on the disposal records is 7875 RA Bridgeford St, Stockton, CA 95206. <br /> <br />CORRECTIVE ACTION: Revise and submit the MWMP to the Environmental Health Department (EHD). The Medical Waste <br />Management Plan must include: <br />(f) The name and business address of the registered hazardous waste hauler <br />(g) The name and business address of the offsite medical waste treatment facility <br /> <br />#24. HSC 118275 - Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be <br />segregated and contained pursuant to this section. The container shall be properly labeled. <br />OBSERVATION: The following containers did not have adequate labeling: <br />1. 1st floor: <br />a. Pharmacy: chemo waste container missing side label (corrected on site) <br />2. 2nd floor: <br />a. Women’s health: procedure room biohazardous waste container missing back label (corrected on site) <br />b. Women’s health: exam room 12 & 14 biohazardous waste containers missing back label (corrected on site) <br />c. Women’s health: exam room 15 biohazardous waste container missing side and back labels <br />d. Women’s health: utility room biohazardous waste container missing side and back labels (corrected on site) <br />e. Adult Family Medicine: room 8 biohazardous waste container missing back label (corrected on site) <br />f. Adult Family Medicine: observation room biohazardous waste container missing back label (corrected on site) <br /> <br />CORRECTIVE ACTION: Submit photos of correction for exam room 15 to the EHD. <br /> <br />#25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br />OBSERVATION: The biohazardous containers in the accumulation storage area were not lined with biohazardous bags. <br /> <br />CORRECTIVE ACTION: Ensure biohazardous containers in the accumulation storage area are lined with biohazardous bags <br />that is ASTM D1709 and ASTM D1922 certified. Submit photos to the EHD. <br /> <br /> <br />Submit evidence of corrections to the EHD by <br />10/18/2025 <br /> <br />Notes: <br />1. Waste is picked up twice a week. <br />3. Biohazardous bags meet ASTM D1709 & 1922 standards. <br />4. Reviewed disposal records from 02/2024 – 09/2025. <br />5. Inspected the following areas: <br />a. 1st floor: flu station near entrance, pharmacy, lab and radiology <br />b. 2nd floor: primary injection, women’s health, pediatrics, adult family medicine <br />c. Accumulation storage area <br />6. This facility generates the following medical waste streams: biohazardous, chemo, pharmaceutical/sharps
