LQG shall maintain tracking and treatment records for 2 years – HSC 117975 <br /> <br />OBSERVATIONS: <br />1. Medical Waste Tracking Forms were missing for the following dates: 4/28/25 and 6/2/25 <br />2. Bill of Lading dated 5/14/24 had a pickup date discrepancy between shipper (generator) and the transporter. <br />3. Bill of Lading dated 4/16/25 had a date discrepancy between the shipper (generator) and the transporter <br />CORRECTIVE ACTIONS: <br />1. Provide proof of tracking and treatment documents for the dates listed above. <br />2 & 3 Provide training to employees on proper maintenance of the medical waste documents. Provide training record to <br />EHD. <br /> <br />23. Biohazard bags shall meet the standards set forth in this section. – HSC 117630 <br />OBSERVATIONS: <br />1. Biohazard bags observed being used in Gowning room 114 and 144A did not meet the specifications of certification for <br />ASTM D1709 and ASTM D1922 <br />CORRECTIVE ACTIONS: <br />1. Provide the proper bags for all biohazard containers within the facility. Bags must meet the specifications for certification <br />of ASTM D1709 and ASTM D1922. <br />31. “Mixed waste” means the mixture of medical and nonmedical waste. Medical waste and hazardous waste is <br />hazardous waste and is subject to regulation as specified in the statues and regulations applicable to hazardous waste. <br />– HSC 117730 <br />OBSERVATIONS: <br />1. Observed a container within the laboratory that had biohazard labels on the container but was not in an approved <br />medical waste container or lined with a biohazard bag. The operator noted that the cartridges that are discarded within <br />the bin is treated as hazardous waste due to corrosivity of reagents. <br />CORRECTIVE ACTION: <br />1. Remove biohazard labels from the hazardous waste container to prevent confusion of contents within the bin. <br /> <br /> <br /> <br />All corrections due to EHD by 7/24/25. <br /> <br /> <br />
