The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for T-Mobile West, LLC SC07221A as of December 02, 2024. <br />Open violations from October 28, 2024 inspection <br />Violation #4 - 1010008 - Business plan reviewed and electronically certified as complete/accurate on or <br />before the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) annually by the due date, January 15. <br />- For 2023, the CERS Submittal was made on 2/16/2023, one month late. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar year <br />and may be submitted beginning November 1 of the previous year <br />CORRECTIVE ACTION: Submit a statement to SJC EHD stating that the business will annually certify/submit the <br />annual HMBP to CERS by January 15. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 1
