There are currently 4 procedure sinks installed. Facility owner intends to install additional sinks in the future. Facility owner is <br /> to contac-the EHE prior to installing additional sinks. <br /> 5. Sterilization cer,ficates were provided for the Metal Mafia needles (lot numbers 220701, 220901 and 23120) Luc<y cartridges <br /> (lot numbers 211211 and 210601) and Stigma cartridges (lot numbers 2024/05, 2024/02 and 2023/08). <br /> 7. The same consent form and medical questionnaire will be used for tattoo and piercing procedures. <br /> 8. The facility has a pool table in the lounge area with an absorbent top. Ensure the pool table is covered when bodw art <br /> procedures are be ng performed. <br /> 9. Facility may not operate and practitioners shall not provide services until the facility is permitted to open and practitioners are <br /> registered with the EHD. <br /> Reinspection on/abot.t: A reinspection fee of$172 per hour may be charged.
