Corrective Action Statement <br /> Violation#5:A current Facility plot plan is maintained and submitted. The UST Monitoring Site <br /> plan does include the location of the leak detectors and is saved on CERS. <br /> Violation#53: We have uploaded a new updated copy of our Financial Responsibilities <br /> document to the CERS website. <br /> Violation#56: We have spoke to our staff and our D.O and will be sure that going forward that <br /> when operators are performing inspections at this facility, they are inspecting all of the required <br /> areas and noting it on the reports going forward. <br /> Violation# 59: I have contacted our D.O and have located all of our missing maintenance and <br /> monitoring records for the last three years. We also spoken to our staff to make sure this does not <br /> happen again moving forward.All current records have been sent to the EHD. <br />
