Box 1394 <br /> Lafayette, CA 94549 <br /> permits@ecochek.com <br /> UST RETROFIT — FULL PERMIT APPROVAL <br /> Re: Kwik Serve Lodi SR2501485 <br /> 420 W Kettleman Ln Ste 1, Lodi, CA 95240 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to do the following on the underground storage tank (UST) system at the above <br /> noted address: <br /> • Install used oil shut-off valve (flapper) <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire district, the San Joaquin Valley Air Pollution Control District, and the local <br /> community development department for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for written approval <br /> rior to commencing work. All new submittals supersede previous ones. <br /> 3. Email notification to ustCcDsigov.orq to arrange the following inspection appointments. All <br /> appointments must be scheduled at least 48 hours in advance. <br /> a. Verification of overfill prevention: Measurement of drop tubes to verify 95% tank volume shut <br /> off. Ensure that all pertinent tank charts and calculations are available for review at the time <br /> of inspection. <br /> The facility cannot receive fuel in the used oil tank prior to the EHD inspection and approval. <br /> 4. Any individual performing the work of a service technician must possess, or work under the direct <br /> and personal supervision of an individual physically present at the work site at all times while <br /> work is being performed who possesses, all of the following: <br /> • One of several permissible valid, current contractor licenses issued by the CSLB <br /> • A current California UST Service Technician certificate issued by the ICC <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> tested, or the developer of the testing equipment or test method(s) being used <br /> 1868 E.
