A test report has not been submitted to the EHD or the California Environmental <br /> Reporting System (CERS). <br /> REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is a (Minor)Violation. <br /> 12 CA Code of Regulations 2637(e), 2637(f) 2010009- Secondary containment test results submitted to the CUPA within <br /> 30 days after the test. <br /> OBSERVATION 1: Secondary containment testing for the 91 sump was performed on 1/15/25 following repairs <br /> (SR2400373). A test report has not been submitted to the EHD or the California Environmental Reporting System <br /> (CERS). <br /> OBSERVATION 2: The 10/29/24 DO inspection report observed on site states that secondary containment retesting was <br /> performed for the 91 sump on 10/4/24. The 12/20/24 DO inspection report observed on site states that the results of the <br /> 10/4/24 secondary containment testing were placed on site. EHD was not notified of the testing and a test report has not <br /> been submitted to the EHD or the California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE: A copy of the 'Secondary Containment Testing Report Form' must be submitted within 30 <br /> days of the test. <br /> CORRECTIVE ACTION: Confirm whether re-testing was completed for the 91 sump on 10/4/24. Immediately provide this <br /> report and the test report for the 1/15/24 testing to the EHD. <br /> This is a (Minor)Violation. <br /> 52 CA Code of Regulations 2635(c)(1) 2030036 - Overfill prevention system has not been overridden and meets overfill <br /> requirements (TCR 9b) <br /> OBSERVATION: Overfill prevention testing was last performed on 6/17/21 and was completed on 7/26/24 for the 87 drop <br /> tube, one month past due. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October 13, <br /> 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed every 36 months thereafter, and <br /> within 30 days of completion of a repair. <br /> This is a (Class 2)Violation. <br /> EHD-CUPA Inspection Report IS02 Page 5 of 7 FA0007787 PRO508452 4517097 6/13/2025 <br /> 1868 E.
