California St. Stockton, CA 95204 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br />application for a permit to do the following on the underground storage tank (UST) system at the above <br />noted address: <br />• Repair to 87 -Siphon Tank STP Sump <br />The permit has been approved with the following conditions: <br />1. Contact the local fire district, the San Joaquin Valley Air Pollution Control District, and the local <br />community development department for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to a: UM <br />for written approval <br />prior to commencing work. All new submittals supersede previous ones. <br />3. Email notification to ust(c�si4ov.orq to arrange the following inspection appointments. <br />All appointments must be scheduled at least 48 hours in advance. <br />a. Integrity testing of the 87 -Siphon STP sump, in accordance with manufacturer <br />specifications. <br />4. Any individual installing underground storage systems or components must possess, or work <br />under the direct and personal supervision of an individual physically present at the work site at <br />all times while work is being performed who possesses, all of the following: <br />• One of several permissible valid, current contractor licenses issued by the Contractors <br />State License Board (CSLB) <br />• A current UST Installation/Retrofitting certificate issued by the International Code Council <br />(ICC) <br />• A valid, current certificate of training from the manufacturer(s) of the component(s) being <br />installed <br />5. Individuals working under the direct supervision of a qualified UST installer or UST service <br />technician must, at a minimum, be employed by the person with the CSLB license and possess <br />all manufacturer certificates of training for the equipment on which they are working. <br />6.
