CA 95209 <br /> Amba Rai Email: arnbsra76 vahoa,com <br /> Call: 408-593-31 9a <br /> Re: SOW for UST Lining <br /> Scope of Work <br /> Prior to the start of the proiecII the USTs shall be emigty of fuel. or as low as passible fleas then 1-inch <br /> from the bottom I. The tank shall be made available prior to the start of the project CGRS shall provide all <br /> equipment, materials. and labor to the following cUtlines the scope of work.- <br /> 1. Prepare health and safety and work plans, procure materials, schedule tasks, and mob to site_ <br /> 2. The Client shalt Dump out and remove all but 14nch of fuel at the tanks bottom, CGRS estimates <br /> no more than 100-gallons of product in the UST prior to our arriving on site. This fuel shall be drummed <br /> up by CORS for the client to dispose of. The tank shall be available when C GR S arrives onsite, <br /> 3. GGRS will excavate a$" X 6' hole and excavate down to the tank top. <br /> 4. CGRS will de-gas the LEST with a forced air venturi dispersing vapors. Once the LEL {lower explosive <br /> limit) is down to zero (0)CG IRS shall prepare the confined space permit and enler each lank for cleanow. <br /> CGRS will use a 4-gas air rrronitor to observe the tanks' conditiall5 every time a confined space certifred <br /> employee is going to enter the tank and every E0 minutes as well_ CGRS crew members retain <br /> oerfifications in 40 Harwoper, Confined Space, Ultrasonic Testing, and I C. A three to four-man crew on- <br /> site at all tines because we are trained a+id have the proper equip mant to complete a safe retrieval of a <br /> crew member if the confined space entrant was to sustain an injury or become unconscious_ Any <br /> rernining petroleum waste shall be drummed for disposal by owner_ <br /> 6. Tanks small be inspected by a certified level 2 A M P P coatings inspector and certified AP1-663 <br /> inspector. The tanks conditions report indicating fit for service shall be prepared for the tank. <br /> E_ The tank will be sandblasted to a near-white metal profile(NACE 21SSPC 5P-10)_ All sandblast media will <br /> be removed from the tank <br /> 7. The tank will be limed using Enviroline 125u high-build epoxy. Once dry (1 -24 hours)the newly installed <br /> liner will be inspected for holidays and dry-flm thickness. <br /> 8. CGRS shall close up the tank using a 24' cover lid, back fill and pour new concrete. C GRS WiII perform a <br /> vacuum test of the annular space Holding -1 Q.0inHg for one hour. GG RS will coordinate all inspections <br /> with CUPA and notify the owner when the tank carp be put back into service. <br /> All worst performed will compty with Title 23 sectlon 266.E&AP11631 "Interior lining and Periodic Inspection <br /> of Underground Storage Tanks", <br /> 8,00 <br />
