SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209)468-3433 Web:www.slgov.org/ehd <br /> CONTINUATION FORM Page: 7 of 15 <br /> OFFICIAL INSPECTION REPORT Date: 1/24/2025 <br /> Facility Address: 2575 Country Club Blvd, Stockton, CA 95204 Program: UST Q <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> 10. 2030074 - Annual Monitoring System Certification Form to the CUPA within 30 days of completion <br /> of the test <br /> OBSERVATION: Annual monitoring system certification and leak detector testing was performed on <br /> 1/31/24 and a test report was submitted on 3/15/24, 14 days late. <br /> REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports <br /> are performed in a timely manner. <br /> 13. 2010017 - Spill bucket testing results submitted to the UPA within 30 days of testing <br /> OBSERVATION: Annual spill container testing was performed on 1/31/24 and a test report was <br /> submitted on 3/15/24, 14 days late. <br /> REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports <br /> are performed in a timely manner. <br /> 24. 2030003 - Leak detection system maintains continuous monitoring that activates an audible and <br /> visual alarm <br /> OBSERVATION: During the January 2023 and January 2024 Monitoring System Certifications, all <br /> vacuum sensors were observed in alarm. They were not corrected until January 2025. An example <br /> includes that the DO documented on multiple reports that multiple sensors were "still in alarm" and had <br /> not been appropriately responded to. Sensors that remain in alarm are unable to communicate new <br /> issues, including leaks, that would otherwise trigger alarms. <br /> REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: Ensure that all alarms are treated as potential leaks and are responded to <br /> immediately. Document that timely and appropriate actions have been taken to resolve monitoring and <br /> maintenance issues and the cause of alarms. Use a properly licensed, trained, and certified contractor <br /> and obtain a permit within one business day from the EHD, if needed.
