OBSERVATION:The UST Facility Operating Permit Application is lacking an active California Department of Tax and Fee Administration (CDTFA) Underground Storage Tank Account Number.The CDTFA number in CERS is 044045933 and has been closed since 9/30/2020. REGULATION GUIDANCE:The application for a permit to operate an underground storage tank shall include the CDTFA registration number issued to the owner of the tank, in accordance with HSC 25286. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS), update the required information, and submit for review by the EHD. This is a repeat violation (Class 2) Violation. 7 CA Code of Regulations 2715(a)(1)(B), 2715(a)(3)2010003-Owner/Operator submitted DO form and/or informed the CUPA of any change(s)within 30 days(TCR 10). OBSERVATION:A new designated operator, David Winkler, began conducting inspections on 3/5/2025 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE:The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation (Class 2) Violation. 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all requirements(TCR 12). OBSERVATION:The Designated Operator failed to check and note all required items on the visual inspection reports.
