The most recent certificate of <br /> financial responsibility document on site is dated 5/25/2023. The most recent copy of the insurance policy observed on <br /> site expired 7/1/24. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the current financial documents to retain on site. Submitted a <br /> corrective action statement to EHD within 30 days. <br /> This is a Repeat(Class 2)Violation. <br /> 59 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly maintained (TCR 9d) <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -Monitoring System Certification test report for 10/29/24 <br /> -Spill Container test report for 10/29/24 <br /> -Monitoring System Certification test report for work done 7/21/25 <br /> -Overfill Prevention test report for used oil drop tube installation 7/21/25 <br /> -Secondary Containment test report for 9/23/25 <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br /> of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br /> and reconciliations. <br /> CORRECTIVE ACTION: The technician on site contacted the CGRS office during the inspection who emailed all missing <br /> test reports to the store. Cashier Malika was able to print out copies and add them to the store's binder. <br /> This was corrected on site. <br /> This is a Repeat(Class 2)Violation. <br /> 67 2712(i) 2030078- Owner/Operator retained a paper or electronic copy of the permit to operate at the facility <br /> OBSERVATION: A current copy of the UST operating permit was not found on site. The most recent permit on site was <br /> dated 2023. <br /> REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be retained at <br /> the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the 2025 permit and retain it on site. <br /> This is a Repeat (Class 2)Violation. <br /> 69 2712(i) 2030033-A current UST monitoring plan has been submitted and is readily accessible at the facility <br /> OBSERVATION: An approved copy of the monitoring plans were not available for inspection. <br /> -The monitoring plan on site lists the overfill prevention method for the used oil tank as audible visual.
