In addition, only the first names <br /> for Paula and Mohammad were listed. Full names should be listed. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate <br /> shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is (Minor)Violation. <br /> 68 CA Code of Regulations 2712(i), 2634(e)2030046- A current UST Response Plan has been submitted. <br /> OBSERVATION:An approved copy of the response plan was not available for inspection. <br /> REGULATION GUIDANCE:A copy of this plan shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is(Minor)Violation. <br /> 69 CA Code of Regulations 2712(i)2030033-A current UST monitoring plan has been submitted and is readily <br /> accessible at the facility. <br /> OBSERVATION:An approved copy of the monitoring plans were not available for inspection. <br /> REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is (Minor)Violation. <br /> Overall Inspection Comments: <br /> Routine UST Inspection <br /> An inspection checklist was provided to the facility operator on the day of inspection, February 12, 2025. A <br /> complete report was finished on March 4, 2025 and replaces the initial checklist. Items#66 and #72 were <br /> removed. Items#1, #52, #56, #68 were added.
