The following is an itemized list of CP22 - UST FULL violations that have not been <br /> addressed for El Dorado Food Mart as of March 25, 2025. <br /> Open violations from August 14, 2024 inspection <br /> Violation#7-2010003-Owner/Operator submitted DO form and/or informed the CUPA of any change(s) <br /> within 30 days(TCR 10). <br /> OBSERVATION:A new designated operator, Dave Winkler, began conducting inspections on June 2024 and <br /> notification was not provided to the EHD within 30 days of the change. <br /> REGULATION GUIDANCE:The owner or operator shall submit a"Designated Underground Storage Tank Operator <br /> Identification Form"no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#10-2030074-Annual Monitoring System Certification Form to the CUPA within 30 days of <br /> completion of the test. <br /> OBSERVATION:A monitoring system certification and leak detector testing was performed on 8/8/2023. A test report <br /> has not been submitted to the EHD or the California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#13-2010017-Spill bucket testing results submitted to the UPA within 30 days of testing. <br /> OBSERVATION: Spill container testing was performed on 8/8/23.
