OBSERVATION: Secondary containment testing was performed on 2/19/25 and a test report was submitted on 5/7/25, 47 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation (Class 2) Violation. 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all requirements(TCR 12). OBSERVATION:The designated operator failed to check and note the correct test dates on the designated operator inspection reports.The secondary containment test date is listed as 2/1/22 on the DO reports for 3/18/25,4/16/25, and 5/14/25. The correct date is 2/19/25. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. -------------------------------------------------------- OBSERVATION: Designated operator reports were not signed within the required time frame. -The 1/22/25 designated operator report was signed by the owner/operator but not dated. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection,the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues.
