Different dates were observed for the same employee for for the years 2021, 2023, 2024. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate <br /> shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a repeat violation (Class 2) Violation. <br /> 69 CA Code of Regulations 2712(i)2030033-A current UST monitoring plan has been submitted and is readily <br /> accessible at the facility. <br /> OBSERVATION:An approved copy of the monitoring plans were not available for inspection. Monitoring plan onsite <br /> does not accurately represent leak detectors in use. <br /> REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a repeat violation (Class 2) Violation. <br /> 77 CA Code of Regulations 2712(f)2030073- Facility has corrected cited violations within 30 days of receiving report. <br /> OBSERVATION:An inspection was last done on 2/27/24 and an inspection report was issued identifying information <br /> to be submitted to bring this site into compliance. This information was required to be submitted within 30 days of <br /> submitting the report. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions.The corrective action shall include all <br /> of the recommendations made in the report.
