CGRS, Inc. is a California Licensed Contractor, <br /> License#803616 ---A/C-61/D40/D64/HAZ. All technicians performing tests and certifications are <br /> individually certified by the equipment manufacturers and hold applicable International Code Council <br /> (ICC) certificates. <br /> 1.0 SCOPE OF WORK: <br /> Task I- Pre-construction activities <br /> • Submit plans and permit application to San Joaquin County Environmental Health <br /> Department (CUPA) for repairs and a cold start permit <br /> • Order parts <br /> • Prepare site for construction as required <br /> Task ll—Spill Bucket& Overfill Valve Installation <br /> • Saw cut, demo, and haul one (1) 4' x 4' area around the diesel spill bucket <br /> • Install Franklin Fueling double wall Defender Series Spill Bucket and Defender Series <br /> Overfill shut off valve on the diesel product tank <br /> • Remove "ball float valve" on the 87 product tank <br /> • Provide Mechanical Leak Detector testing of prior contractor installation <br /> • Provide pretest of spill bucket <br /> • Place#4 rebar 18" on center each way and pour back 2500 PSI concrete <br /> Task III— Finalize project <br /> • Call for required inspections & provide OPI Inspection/testing <br /> CGRS anticipates onsite construction will take 2-3 days. <br /> Please call or email me should you have any further questions regarding this Scope of Work. <br /> Submitted by: CGRS, Inc. <br /> CGRS, Inc. 5444 Dry Creek Rd. <br /> Sacramento, California 95838 <br /> Ades 7;;#,Oraa Cell (626-627-8316) <br /> Matt Thomas State of CA Contractors License#803616 <br /> Business Development Class "A/C61/D40/D63/HAZ" — HAZWOPER Certified <br />
