The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for STKN MUD WELLS #18 as of February 03, 2025. <br />Open violations from May 10, 2024 inspection <br />Violation #3 - 1010017 - Business plan readily available to site personnel responsible for emergency <br />response or training. <br />OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon request. <br />Employees do not have access to the Emergency Response Plan and the inventory information on-site. <br />REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required by <br />this article, and the site map required by this section shall be readily available to personnel of the business or the <br />unified program facility with responsibilities for emergency response or training pursuant to this section. <br />CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete, the following elements are missing: <br />- No loading area for hazardous material <br />- Hazardous material on inventory list is not shown on map <br />- Chlorine was shown on map, chlorine is not included in inventory and chlorine was not on site. <br />- No evacuation staging area <br />- Emergency Response equipment location is not indicated <br />- Emergency shut off not indicated <br />- Access/Exit points not indicated <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 2
