The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for STKN MUD WATER WELL#20 as of February 03, 2025. <br /> Open violations from May 10, 2024 inspection <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#10-1010004-Complete and accurate Hazardous Materials Inventory information electronically <br /> submitted. <br /> BSERVATION: 55 gal drum of used oil found on site that have not been reported to CERS. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#11 -1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION: The site map is incomplete, due to the following missing elements: <br /> -Map indicates chlorine is on site, chlorine was not on site and not included in the inventory. <br /> -Map does not indicate location of eyewash station/shower. <br /> -Map does not indicate Loading zone for hazardous materials. <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Page 2 of 3 <br />
