The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for CHURCHS CHICKEN #5773 as of July 21, 2025. <br /> Open violations from January 02, 2025 inspection <br /> Violation#11 -1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION:A site map was not completed and submitted as part of the business plan. <br /> -A site map with a1I required content was not available on site.Additionally, In CERS the facility has selected the <br /> option"Stored at Facility",which is not allowed and will lead to the submittal being"NOT ACCEPTED". <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets,access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment,and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs,as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System(CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ll�/This violation was corrected ❑This violation will be corrected by(date): <br /> El Supporting documents included <br /> Describe ctions taken or will be taken to correct violation: <br /> d+t <br /> Violation#14-1020002-Provided initial and annual training and maintained training records for a <br /> minimum of three years. <br /> OBSERVATION:The training records available were for Carlos Salazar for the year 2024, no other records were <br /> available to showcase training for all employees for the last 3 years. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: (A)Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B)Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br /> notice,for the business site.
