A map was updated and uploaded into CERS during the <br />inspection. <br />This is (Minor) Violation. <br />CA Health and Safety Code 25505(a)(3) 1010010 - Electronically submitted adequate response plan/procedures for <br />release/threatened release of hazmat. <br />OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. The Consolidated Emergency <br />Response Plan / Contingency Plan was missing: <br />- CUPA phone number <br />- Hospital phone number <br />- Section H. Earthquake Vulnerability was not completed <br /> <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release <br />or threatened release of hazardous materials, including, but not limited to, the following: - immediate notification of <br />local emergency personnel and the Environmental Health Department (EHD) - procedures for the mitigation of a <br />release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br />evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: This was corrected on site. The Consolidated Emergency Response Plan / Contingency Plan <br />was updated and uploaded in to CERS during the inspection. <br />This is (Class 2) Violation. <br /> 12 <br />Overall Inspection Comments: <br />Routine HMBP Inspection <br />An inspection checklist was provided to the facility operator on the day of inspection, January 29, 2025. A <br />complete report was finished on February 10, 2025 and replaces the initial checklist. <br />All violations were corrected on site. No additional follow up is required. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($172).
