A test report has not been submitted to <br /> the EHD or the California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE:A copy of the 'Secondary Containment Testing Report Form'must be submitted within 30 <br /> days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is a repeat violation (Class 2) Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11)2010007-Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance(TCR 11). <br /> OBSERVATION: Financial responsibility documents have not been submitted to the EHD. The most recent financial <br /> responsibility documents in CERS are dated 11/4/22. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports. <br /> The incorrect and missing information included the following from the inspection reports: <br /> -Designated operator reports were not signed within the required time frame. -The 3/22/24 designated operator report <br /> was not signed by the owner/operator. <br /> -The designated operator failed to check and note the correct test dates for the Overfill Protection Equipment on the <br /> designated operator inspection reports.The correct date for Overfill Protection Equipment is 6/6/24. <br /> -The 1/11/25 DO report did not list the correct inspection date and signature date. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing <br /> and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all <br /> of the required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site: <br /> -Leak detection results on 11/24/24 <br /> FA0007486 PR0506538 SCO01 03/06/2025 <br /> EHD Rev.12/06/2021 Page 6 of 8 CP22-UST FULL OIR <br /> 1868 E.
