Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />May 02, 2024 <br /> Facility Address: <br /> 2615 Station Dr, Stockton, CA 95215 <br /> Facility Name: <br />Sunview Logistics/ Speed Intermodal Inc <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: This was corrected on 06/18/2024. <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25505(a)(4) 1020001 - Established and electronically submitted adequate training <br />program. <br />OBSERVATION: A training program was lacking. <br />REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, shall <br />include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br />release of a hazardous material. Training shall cover, but not limited to, all of the following: (A) Immediate notification <br />contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures <br />for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or <br />the environment. (C) Evacuation plans and procedures, including immediate notice, for the business site. These <br />training programs may take into consideration the position of each employee. <br />CORRECTIVE ACTION: This was corrected on 06/18/2024. <br />This is (Class 2) Violation. <br /> 13 <br />CA Health and Safety Code 25505(a)(4) 1020002 - Provided initial and annual training and maintained training records <br />for a minimum of three years. <br />OBSERVATION: The facility could not provide training records at the time of the inspection. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br />initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br />threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br />procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br />unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br />harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br />notice, for the business site. This training shall be documented electronically or by hard copy and shall be made <br />available for a minimum of three years. <br />CORRECTIVE ACTION: This violation was corrected. 06/05/2024 training records were received. <br />This is (Class 2) Violation. <br /> 14 <br />SJC Ordinance Code 4432 1015 - Administration/Documentation - General Local Ordinance. <br />OBSERVATION: Facility failed to pay annual operating fees for the year 2024. <br />ORDINANCE: Fees shall be paid once a year at the time of submittal of an inventory form. <br />CORRECTIVE ACTION: Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton <br />Avenue, Stockton, CA 95205.
