The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for SUBWAY #25225 as of December 20, 2024. <br />Open violations from July 22, 2024 inspection <br />Violation #3 - 1010017 - Business plan readily available to site personnel responsible for emergency <br />response or training. <br />OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon request. <br />REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required by <br />this article, and the site map required by this section shall be readily available to personnel of the business or the <br />unified program facility with responsibilities for emergency response or training pursuant to this section. <br />CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #4 - 1010008 - Business plan reviewed and electronically certified as complete/accurate on or <br />before the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) ANNUALLY by the due date, January 15. <br />- A CERS Submittal has not been made since 5/16/2024 <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar year <br />and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 3
