Per our conversation, Costco is planning to expand its fuel facility in Lodi, <br />which will involve the installation of seven (7) dispensers, TMS Additive Injection System, turbine sumps, TLS-450 <br />monitoring system, and all associated piping. My understanding is that your agency requires a UST Permit to <br />Construct and Operating Permit. I have a few questions just to confirm Costco meets all of the CUPA’s <br />requirements. <br /> <br />1. I have reviewed your website and found two forms (Application Form and UST Installation Form) to apply <br />for the UST Permit to Construct. I understand that these can be submitted online along with plans, cut <br />sheets, contractor’s certifications, tank charts, buoyancy calculations, and ELD work plan. Could you <br />please confirm that this is correct? <br />2. No tanks will be removed, but there will be some removal of product piping involved in this project. Does <br />your agency require a UST Closure Application and Permanent Closure Report for the piping removal? <br />3. Your website states that the maximum review time for plan review is 15 working days. Could you please <br />confirm that this is still the estimated review timeline per your agency’s current workload? <br />4. Are there any other requirements with your agency that we should be aware of prior to construction start? <br /> <br />We have already reached out to the other state and local agencies to confirm their requirements separately.
