The following is an itemized list of CP22 - UST FULL violations that have not been <br /> addressed for TRACY SHELL as of March 03, 2025. <br /> Open violations from January 08, 2025 inspection <br /> Violation#11 -2030048-Sec containment testing conducted per requirements&records kept for 36 mths <br /> (TCR 9d). <br /> OBSERVATION: The 4/14/2022 Secondary containment test results contained failed test results for all the <br /> components that were tested. <br /> The 5/22/2024 Secondary Containment test result were missing piping certs, no CERS ID, did not identify the specific <br /> diesel sump that was tested, all the components were failed during the 4/15/2022 Secondary Containment test were <br /> not tested. <br /> REGULATION GUIDANCE: Secondary containment testing is required to be performed once every 36 months. <br /> CORRECTIVE ACTION:Take all necessary precautions to ensure testing is performed in a timely manner. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#12-2010009-Secondary containment test results submitted to the CUPA within 30 days after the <br /> test.. <br /> OBSERVATION: Secondary containment testing was performed on 5/22/2024 and a test report was submitted on <br /> 7/17/2024, 55 days late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION:Take all necessary precautions to ensure testing and submittal of test reports are performed <br /> in a timely manner. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 2 <br />
