S A N J O A Q U I N Environmental Health Department <br /> CP22 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> QUIK STOP MARKET#132 3555 W HAMMER LN, STOCKTON April 01 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> recorded. <br /> OBSERVATION:The work order dated 6/6/2024 states that gas was found in the sump as a result of a broken siphon <br /> line on top of the leak detector in the 87 STP sump.All required information regarding this release from the primary <br /> containment was not recorded. <br /> REGULATION GUIDANCE:Any release from the primary containment that does not escape the secondary <br /> containment,does not increase the hazard of fire or explosion,does not cause any deterioration of secondary <br /> containment, and was cleaned up within eight hours of being detected or when it should reasonably have been <br /> detected,shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types,quantities,and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment <br /> CORRECTIVE ACTION: Immediately record this release with the information required, and maintain it on site with the <br /> facility's monitoring records. Submit a copy of the recordable release statement to the EHD by 5/1/2025. <br /> This is(Class 2)Violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 1, 2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. <br /> Starting July 1, 2024, all in-office CERS help will be provided at EHD hourly rate ($167).
