SANsJOAQUIN Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Phone: Date: <br /> EXCEL PETROLEUM - HAMMER LANE 1(209) 648-9990 1 June 02, 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance (TCR 11) <br /> OBSERVATION: Current financial responsibility documents were not found on site. <br /> REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a Repeat(Class 2)Violation. <br /> 56 CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br /> requirements (TCR 12) <br /> OBSERVATION: Designated operator reports were not signed within the required time frame. <br /> -The designated operator reports for 9/20/24, 10/18/24, 11/15/24, 12/13/24, 1/9/25, 2/7/25, 3/7/25, 4/4/25, 5/2/25, and <br /> 5/30/25 have not been signed or dated by the owner/operator. <br /> REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the <br /> designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of <br /> being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action <br /> taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required <br /> time frame in the future. <br /> OBSERVATION: The designated operator failed to check and note the correct test dates for the secondary containment <br /> testing on the designated operator inspection reports. The dates are missing from the 8/23/24, 9/20/24 and 10/18/24 DO <br /> reports.
