Work orders for <br /> the dispenser replacement and secondary containment testing conducted on 8/12/2025 were not found on site. <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all <br /> readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br /> This is (Class 2)Violation. <br /> 77 CA Code of Regulations 2712(f)2030073- Facility has corrected cited violations within 30 days of receiving report. <br /> OBSERVATION:An inspection was last done on 8/13/2024 and an inspection report was issued identifying information <br /> to be submitted to bring this site into compliance. This information was required to be submitted by 9/21/2024. This <br /> information was received on 10/9/2024, 18 days late. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include all <br /> of the recommendations made in the report. The operator shall submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is (Minor)Violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 9/13/2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> FA0002324 PR0231223 SCO01 08/14/2025 <br /> EHD Rev.12/06/2021 Page 6 of 7 CP22-UST FULL OIR <br /> 1868 E.
