This information was required to be submitted 30 days after receiving the inspection <br /> report. This information has not been received resulting in a non-compliant status for this facility. <br /> The outstanding violations are itemized on the following pages and may include violations from past <br /> inspections. Complete and submit a copy of the Return to Compliance Certification form to the EHD <br /> with a statement documenting the corrective actions that have been or will be taken for each violation, <br /> and any supporting paperwork. <br /> Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br /> billed at the current hourly rate (currently $172, moving to $179 starting on July 1st).
