The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for SUBWAY SANDWICHES #23091 as of March 05, 2025. <br />Open violations from October 21, 2024 inspection <br />Violation #4 - 1010008 - Business plan reviewed and electronically certified as complete/accurate on or <br />before the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) annually by the due date, January 15. <br />-No CERS submittal was made for 2024 <br />-No CERS submittal was made for 2023 <br />-2022 CERS submittal was made late on 4/21/2022 <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar year <br />and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete. <br />-Emergency evacuation staging area label is missing <br />-Fire extinguisher location label is inaccurate <br />-Loading area label is missing <br />-Storm and sewer drain labels are missing <br />-Emergency shutoff labels are missing (gas, water, electricity) <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 2 of 3
