The site map is missing the evacuation staging location, storm drain <br />locations, emergency shut offs for the refrigerant system, utilities shut offs for gas water & power, chemical locations, <br />and adjacent street locations. A site map key or legend must be used when using symbols or acronyms. Use a site <br />map key to identify the red dots on the site map. All of the elements above need to be reflected on the updated site <br />map. The updated site map can be uploaded in CERS and new submittal made. <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is (Minor) Violation. <br />12 <br />CA Health and Safety Code 25505(a)(3) 1010010 - Electronically submitted adequate response plan/procedures for <br />release/threatened release of hazmat. <br />OBSERVATION: The emergency response(ER) procedures were not complete. The emergency response and <br />contingency plan has the wrong phone number listed as local Unified Program Agency (UPA) phone number. List <br />209-468-3420 as the local UPA phone number on line C 1. The location for the telephones, intercom, and portable <br />radios were not listed in section G. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: - immediate <br />notification of local emergency personnel and the Environmental Health Department (EHD) - procedures for the <br />mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the <br />environment - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br />EHD Rev. 12/06/2021 <br />FA0018047 PRO548500 SCO01 01/10/2025 <br />Page 2 of 3 CP22 - HMBP OIR <br />1868 E.
