If a sample was not collected for the month(s) of April 2010, <br />you are being directed to provide public notification in accordance with the enclosed methods and to submit a <br />copy of the completed notice to EHD, within ten days. <br />If bacteriological samples were collected, a copy of the lab results should be sent to our office as soon as <br />possible. Results can be mailed to the above address or faxed to (209) 468-0341. Public notification will not <br />be required if the results meet State standards. Analytical results of all required samples collected for a <br />system in a calendar month shall be reported to this office no later than the tenth day of the <br />following month. <br />Please be aware that you may be billed $230 (a minimum of 2 hours at a rate of $115 per hour) for this and <br />any further enforcement activity required by the EHD to obtain the sample results, documents, or actions <br />the EHD has requested within this letter. Failure to submit the required forms and results will result in a <br />Procedural Violation that is billed at a two hour minimum rate of $230.
