After completing the inspection, an official inspection report was <br />forwarded to AT&T citing the following issues that were not in compliance with the County’s regulations: <br />-A new Sample Siting Plan was not completed for 2010; <br />-Backflow prevention testing had not been completed for the year 2009; <br />-The RP that was located on the east wall of the car wash had been removed, as had the vacuum <br />breakers at the hose bib; and <br />-The contract information for the new CDO of record at the site had not been sent to the County. <br />On February 24, 2010, Shaw oversaw Quality Services, Inc, as they conducted backflow testing at the <br />site. While there, vacuum breakers were installed at the hose-bib in the car wash and the location of the <br />former RP was investigated. It was determined that confirmation from you would be required to ensure <br />that the correct device was installed at the correct location. On the following day, February 25, 2010, I <br />called you and confirmed the location and the type of device that would be required. <br />In the time that has elapsed since our conversation, Shaw has been working with Quality Services, Inc, in <br />an effort to order the required parts and install the RP device. I am hoping that the work can be performed <br />by the end of this week, or possibly next week. Per our phone conversation this morning, March 9, 2010, <br />you acknowledged that you were ok with extending the deadline past the original date of March 10, 2010, <br />as long as we were making a good faith effort to ensure that the work is completed. Thank you for your <br />understanding and for granting us a grace period beyond the original deadline to have the work <br />Upon receipt of this inspection report, Shaw completed the sample siting plan and submitted it to your <br />office via US mail.
