JAN 1 4 2009 <br />January 12, 2008 <br />RE: <br />Dear Mr. Wong: <br />PVB and RP devices require annual testing. <br />Thank you for your help concluding this matter. <br />We understand that in your discussion with our consultant, Rob Delnagro, of Shaw <br />Environmental on or about 1-5-09, you said that all of the above requests have been satisfied <br />and that there is no further action required of us related to this inspection. Please contact me <br />immediately at any of the numbers below if there is any misunderstanding reflected herein, or if <br />you need anything further from us. <br />This letter is to confirm your verbal approval of our corrective action that resulted from your <br />inspection referenced above. The requested action items were: <br />VI.S. has no CDO at this time. Submit a copy of CDO certification of <br />staff to EHD, or a copy of a professional CDO contract to EHD by <br />April 21, 2008. <br />Demonstrate to EHD there is no cross connection (such as if water is tied <br />to the system after the RP device, or not), or install a RP, or provide air <br />gap, as backflow prevention measure by 4/21/08 <br />Four quarters of gross radiological monitoring have not been <br />completed.
