Repeat notice by hand shall <br />be given every 3 months for as long as the violation or failure exists. <br />(Continuous posting of Notice of Water Quality Failure) - Notice by posting in <br />conspicuous places within the area served by the system shall be initiated within 14 days after <br />the violation or failure. Posting shall continue for as long as the violation or failure exists. <br />Provide the Environmental Health Department (EHD) with a completed copy of the required public <br />notification and the methods you are using within ten (10) days of receipt of this letter. After a <br />positive, four repeats must be taken WITHIN 24 HOURS OF BEING NOTIFIED by your <br />laboratory and five routine samples shall be taken the following month. Please be aware that <br />bacteriological results collected for a system in a calendar month shall be reported to the EHD no later <br />than the tenth day of the following month. <br />Submit to the EHD within 30 days information on the current status of physical works and operating <br />procedures, which may have caused the elevated bacteriological findings, or any information on <br />community illness suspected of being waterborne and corrective actions taken to correct the violation. <br />This shall include, but not limited to: <br />A. Operating procedures that are <br />count; <br />B. Any interruptions in the treatment process; <br />C. System pressure loss to less than 5 psi (which will occur during a loss of power), <br />D. Vandalism and/or unauthorized access; <br />E. Physical evidence indicating bacteriological contamination of facilities; <br />F. Analytical results of any additional samples collected, including source samples; <br />G. Community illness suspected of being waterborne; <br />H.
