ACTIONS REQUIRED <br />PBWS is hereby directed to take the following actions: <br />Response: Sample results will continue to be submitted to Mr. Borges as directed. <br />Response: Samples taken on December 18, 23 and 31,1996 are attached <br />(Attachment 2). Although all samples were coliform absent, only one sample was <br />taken on December 18,1996 and no samples were taken the week of January 6, <br />1997. Weekly sampling of the well head and distribution system tap was resumed <br />on January 15,1997 and will continue until the requirement for three consecutive <br />weekly sampling events that are coliform absent has been met. <br />Response: Well head sample collected September 19,1996. Laboratory analysis <br />completed October 1,1996 (Attachment 3) <br />Response: Technical assistance provided by G. Oliveira, Sr. REHS on September <br />17,1996 (see Attachment 1-Inspection Report). Corrections identified in the <br />Inspection Report were completed as follows: <br />• Public notification for an August 1996 procedural violation conducted on <br />September 18,1996 (Attachment la) <br />• New water sampling tap installed and samples taken on September 16,1996 <br />(Attachment lb) <br />• Required repeat samples (four) taken on September 16 and September 19, <br />1996 (Attachment lb & 1c) <br />• System disinfecting performed on September 23,1996 and November 7 <br />and November 20,1996 (Attachment Id) <br />2. Collect and have analyzed one (1) well head bacteriological sample and one (1) <br />distribution system bacteriological sample per week for a minimum of three (3) <br />consecutive weeks until obtaining three consecutive weekly sampling events that are <br />coliform absent. <br />3. By January 5,1997, have a state-certified analytical laboratory collect well head <br />samples and have them analyzed for nitrate, nitrite, and inorganic chemicals as <br />required by CCR, Title 22, Sections 64432(b)(1) and 64432.l(a)(b). <br />1. Obtain technical assistance and investigate to determine the cause of the <br />bacteriological failures and report the results of the investigation to the Department <br />within thirty (30) days of receipt of this citation. Have the well and storage tank <br />properly disinfected as directed. (Meeting of September 17,1996). <br />4.
