37 <br />38 <br />39 <br />40 <br />41 <br />42 <br />43 <br />44 <br />45 <br />46 <br />47 <br />48 <br />49 <br />50 <br />51 <br />52 <br />53 <br />54 <br />55 <br />ACTIONS REQUIRED56 <br />PBWS is hereby directed to take the following actions:57 <br />58 <br />(1)59 <br />60 <br />61 <br />62 <br />63 <br />64 <br />65 <br />66 <br />67 <br />PBWS failed to monitor its source for inorganic chemicals and report the <br />results to the Department, Section 64432(b)(1). <br />In accordance with Section 116650(e) of the H & S Code, these violations are <br />classified as continuing. <br />PBWS failed to collect and have analyzed five (5) routine samples during the <br />month of August 1996, Section 64423(c). <br />PBWS failed to monitor its source for nitrite and report the results to the <br />Department, Section 64432.1(b). <br />PBWS failed to monitor its source for nitrate and report the results to the <br />Department, Section 64432.1(a). <br />Obtain technical assistance and investigate to determine the cause of the <br />bacteriological failures and report the results of the investigation to the <br />Department within thirty (30) days of receipt of this citation. Have the well <br />and storage tank properly disinfected as directed. (Meeting of September <br />17, 1996). <br />In accordance with Section 116650 (e) of the H & S Code, these violations are <br />classified as noncontinuing.
