OBSERVATION:The designated operator failed to check and note the correct test dates on the designated operator inspection reports: -The 3/25/25 and 4/22/25 DO inspection reports listed the Overfill Prevention Equipment date as 4/8/24. The correct date is 3/4/25. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation (Class 2) Violation. 57 CA Code of Regulations 2715(c)2030012-Initial and annual training provided to employees and at least one trained employee is present. OBSERVATION:The designated operator employee training was not performed within 12 months of the previous training date.The most recent training certification found on site was dated 1/27/24. REGULATION GUIDANCE:The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
