Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br /> of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br /> and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 85 CA Code of Regulations 2631(a); HSC 25290.1(c) 2060027-Tank system primary containment is constructed, <br /> operated, and maintained product-tight. <br /> OBSERVATION: Liquid was observed in the UDC's of dispensers 5/6, 8/9, 11/12 and of satellites 8/9 and 11/12, <br /> indicating a leak in the primary containment. <br /> REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a (Class 2)Violation. <br /> Overall Inspection Comments <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br /> by 7/6/25. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. Starting July 1, 2024, all in-office CERS help will be provided at EHD hourly rate ($172).
