SAN JOAQUIN Environmental Health Department <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Phone: Date: <br /> SMK CHEVRON 1(209) 333-1740 Mav 30, 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 1 CA Health and Safety Code 25284; CCR 2711(d) 2030021 - Facility has a valid Permit to Operate from the CUPA <br /> OBSERVATION: A permit to operate the UST system is not available on site <br /> REGULATION GUIDANCE: No person may own or operate an UST unless a permit for its operation has been issued by <br /> the local agency to the owner or operator of the UST system. <br /> CORRECTIVE ACTION: Immediately obtain a permit to operate a UST system from the EHD. <br /> This is a (Minor)Violation. <br /> 50 CA Code of Regulations 2635, 2637.1, 2665, 2712 2060020- Spill buckets meet requirements, have been tested as <br /> required, and records kept for 36 mths (TCR 9a) <br /> OBSERVATION: The Diesel spill container failed when tested. <br /> REGULATION GUIDANCE: All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed, trained, and certified contractor to repair or replace the <br /> spill container under permit and inspection of the EHD. <br /> This is a (Minor)Violation. <br /> Overall Inspection Comments <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br /> by June 29,2025. To minimize person to person contact EHD is choosing to write the name of person receiving the <br /> report instead of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at the EHD <br /> hourly rate ($172). To schedule an appointment, please call (209) 468-3420.
