The missing alarms include: <br /> -8/28/24, Q2: 87 PLLD-Gross Line fail, S1: 91 Product Line- No Vacuum alarm, S2: 91 Vent Line-no vacuum alarm <br /> -8/29/24, Q1: 91 PLLD-Gross Line Fail, S1: 91 Product Line- No Vacuum alarm <br /> -9/30/24, Q1: 91 PLLD-Gross Line Fail <br /> -10/23/24, L14: Dispensers 11/12-low liquid alarm <br /> -10/24/24, L14: Dispensers 11/12-low liquid alarm <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> OBSERVATION 2: The designated operator failed to check and note all required items on the visual inspection reports. <br /> The incorrect and missing information included the following from the inspection reports: -The DO marked N/A on the <br /> DO reports the month following compliance items not being followed up on. - Facility failed to provide a response on <br /> the DO report to compliance issues documented by the DO. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing <br /> and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all <br /> of the required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site.
