Hazelton Avenue | Stockton, California 95205 | T 209 468-3420 | F 209 464-0138 | www.sjgov.org/EHD <br />The "Initial Training Date" column listed on the Facility <br />Employee Training Certificate is confusing and we suggest renaming the column to read something like "Date of Initial Training". <br />As was noted during the inspection, employees who received the training placed the initials of their name in this column. We <br />take issue that violation was issued for not maintaining the Employee Training Certificate. None the less, the date that each <br />each individual received their initial training was added to the form and the form is attached to document this action.
